About The Position
Concierge Home Care is hiring a Home Health Physical Therapist Assistant (PTA). This role offers flexibility, independence, and the ability to increase earnings while providing meaningful one-on-one care in the patient's home. The coverage area is Bradenton, FL, with territories designed to minimize drive time through clustered scheduling. The position operates on a pay-per-visit model with flexible scheduling options, including full-time, PRN, or modified schedules, allowing for greater control over work-life balance. The company utilizes technology such as AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) to reduce documentation time, enabling clinicians to spend more time with patients. There are also opportunities for growth and clinical support, including structured onboarding, mentorship, a preceptorship program, ongoing clinical training, free CEUs, and career advancement opportunities.
Requirements
- Active, unencumbered Florida Physical Therapist Assistant (PTA) license
- Valid driver’s license, reliable transportation, and auto insurance
- Employment is contingent upon successful completion of an AHCA Level 2 background screening.

Responsibilities
- Provide hands-on therapeutic care following the plan of care established by the Physical Therapist
- Assist patients in improving mobility, strength, and independence
- Collaborate with patients, caregivers, and interdisciplinary teams
- Monitor patient progress and communicate updates to the supervising PT
- Complete documentation using streamlined, AI-assisted tools
