About The Position

Elite Home Health is a Medicare-certified home health agency seeking a dedicated Physical Therapist Assistant (PTA) to provide skilled therapy services to patients in their homes. The PTA will work under the direction and supervision of a licensed Physical Therapist (PT) to help patients regain mobility, maintain independence, and improve their quality of life. This role is ideal for a compassionate clinician who values teamwork, patient-centered care, and meaningful outcomes.

Requirements

Graduate of an accredited Physical Therapist Assistant program
Current Florida PTA license/certification in good standing
Current CPR certification
Valid driver's license, reliable transportation, and auto insurance
Ability to meet all required background and compliance requirements

Nice To Haves

Previous clinical experience preferred
Home health experience a plus

Responsibilities

Provide physical therapy treatments in accordance with physician orders, the established therapy plan of care, PT direction, agency policies, and applicable regulations.
Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and mobility training.
Support strengthening, fall prevention, and safe functional movement.
Monitor patient response to treatment and report progress or concerns to the PT.
Reinforce patient and caregiver education as directed by the PT.
Complete accurate and timely documentation in the EMR.
Communicate with the PT and interdisciplinary care team regarding patient needs.
Participate in care coordination, meetings, in-services, and required training.
Follow Medicare requirements, state regulations, and agency policies.
