About The Position

HealthPro Heritage at Home is hiring a PRN Physical Therapist Assistant to serve patients throughout Chagrin Falls/Solon/Aurora/Northfield/Macedonia/Garfield Heights, OH & Surrounding Area. This role offers flexibility, strong pay, and a supportive team environment. The PTA will provide skilled Physical Therapy Assistant services in patients' homes, helping them regain independence and confidence in their daily lives. The position is ideal for supplementing income or maintaining independence in one's schedule, with no weekends required.

Requirements

Ohio licensure as a Physical Therapist Assistant (or eligibility for licensure)
A passion for one-on-one patient care in a home setting
Strong clinical judgment and the ability to work independently
Reliable transportation for visits throughout the service area
Associate’s degree in Physical Therapist Assisting from an accredited program.
Valid state licensure as a Physical Therapist Assistant, or license eligible
Strong assessment, diagnostic, and therapeutic abilities with excellent communication and interpersonal skills.
Proficient in using diagnostic tools, therapy equipment, relevant software, and electronic health record systems, with a proven ability to work collaboratively in a multidisciplinary team.

Responsibilities

Assist in implementing treatment plans designed by Physical Therapists.
Provide direct patient care including therapeutic exercises, manual therapy, and modality applications.
Monitor and document patient progress, report observations to the Physical Therapist, and educate patients and families on treatment protocols, exercises, and self-care techniques.
Accurately document patient treatments, progress, and responses in compliance with facility policies and regulatory standards.
Work closely with Physical Therapists, other healthcare professionals, and support staff to ensure coordinated patient care and effective treatment outcomes.
