About The Position

Creative Health Solutions (CHS) is seeking a passionate Physical Therapist Assistant (PTA) to join their team at a skilled nursing facility with assisted living in Winter Haven. CHS is a therapist-operated, in-house team that prioritizes clinical excellence, collaboration, and patient-centered care. This role offers the opportunity to work in a modern, well-equipped facility with state-of-the-art rehab technology and provide quality, patient-focused care in both skilled nursing and assisted living settings. Valid State PTA License

Responsibilities

Implement treatment intervention plans to address the goals on the plan of care.
Work closely with a team of healthcare professionals to create personalized treatment plans for patients.
Document patient progress and communicate with healthcare professionals regarding treatment plans and patient outcomes.
Continuously educate patients and their families on therapeutic procedures and techniques.
Stay up to date with the latest therapy techniques and methodologies.
