About The Position

RVNAhealth is seeking compassionate, motivated Physical Therapist Assistants (PTAs) to join their growing In-Home Outpatient Rehabilitation (IHOP) team. In this unique role, you will deliver outpatient physical therapy services in patients' homes and senior living communities, providing personalized, one-on-one care that helps individuals maintain their independence and quality of life. This role offers flexibility, autonomy, and support from one of Connecticut's most respected nonprofit healthcare organizations. Opportunities are available throughout three territories: South (Stamford, Greenwich, Darien, New Canaan, Norwalk, Westport), Central (Ridgefield, Redding, Danbury, Wilton), and North (Danbury, New Milford, Brookfield, Bethel, Newtown). The territory-based model helps minimize travel while allowing you to become a trusted rehabilitation provider within the communities you serve. As an In-Home Outpatient Physical Therapist Assistant, you will work under the direction of a licensed Physical Therapist to provide skilled outpatient rehabilitation services under Medicare Part B. You will help patients recovering from orthopedic procedures, neurological conditions, chronic illnesses, and age-related mobility challenges improve their function and safely remain active in their homes and communities.

Requirements

  • Associate degree from an accredited Physical Therapist Assistant program
  • Current Connecticut Physical Therapist Assistant license
  • Reliable transportation and a valid driver's license
  • Passion for helping patients achieve their highest level of function

Nice To Haves

  • Outpatient, home health, or rehabilitation experience preferred, but not required
  • Strong communication, organization, and interpersonal skills
  • Ability to work independently while collaborating with a multidisciplinary rehabilitation team

Responsibilities

  • Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist
  • Implement individualized treatment plans designed to improve strength, balance, mobility, endurance, and functional performance
  • Assist patients with gait training, transfers, therapeutic exercise, and functional mobility
  • Educate patients and caregivers on home exercise programs, safety strategies, and adaptive techniques
  • Monitor patient progress and communicate clinical updates to the supervising Physical Therapist
  • Collaborate with physicians, therapists, and other healthcare professionals to support optimal patient outcomes
  • Complete timely, accurate electronic documentation in accordance with Medicare Part B and agency standards
  • Deliver compassionate, patient-centered care that promotes long-term wellness and independence
