About The Position

Exceptional Rehab is seeking a PRN Physical Therapist Assistant (PTA) for their ST CHARLES LOCATION IN COVINGTON KY. This role is for a passionate Physical Therapist Assistant (PTA) looking to make a difference while enjoying a flexible schedule. Exceptional Rehab is an in-house therapy provider for Exceptional Living Centers, specializing in offering physical, occupational, and speech therapy services for skilled nursing centers throughout the United States. All partner facilities are equipped with a skilled nursing unit to allow for more focused rehabilitation. This is an opportunity to be part of a team that makes a difference in residents' lives.

Requirements

  • Licensed PTA in the state the facility is located in
  • Strong communication and teamwork skills
  • A passion for helping others thrive!
  • A graduate of an approved education course for physical therapy assistants.
  • Licensed, if applicable, in the state in which they will be practicing.

Nice To Haves

  • Prior long-term care and EMR experience strongly preferred.
  • Driver’s license per position requirements.

Responsibilities

  • Provides therapy and performs selected physical therapy interventions under the direction and supervision of the Physical Therapist.
  • Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department.
  • Obtains data related to the interventions provided.
  • Makes modifications in selected interventions either to progress the resident as directed by the physical therapist or to ensure resident safety and comfort.
  • Demonstrates knowledge and appropriate application of precautions and contraindications to physical therapy treatment procedures and modalities.
  • Carries out the d/c plan as determined by the PT.
  • Maintains proper documentation, including creating and maintaining a resident record for each patient treated; completing documentation per policy including timeliness and medical necessity support; assures therapy billing and labor entries are complete, accurate and timely.
  • Communicates effectively by maintaining communication with supervisory PT; participating in supervisory visits with the supervising PT as required by the State Practice Act; requesting reexaminations from the supervising PT when a change in resident status is identified; partnering with multi-disciplinary team to carry out plan of care and advocate for resident and communicating with the interdisciplinary team regarding treatments and discharge planning.
  • Relates professionally, positively, and cooperatively with residents, family members, and other employees at all levels.
  • Participates in quality and process improvement activities, as directed.
  • Participates in continuous survey readiness, as directed.
  • Maintains and applies current skills and knowledge through continuing education and in-service programs.
  • Maintain knowledge of Medicare Policies and Regulations, LCDs, and NCDs that directly impact care.
  • Participates in in-service training and completes all mandatory training within designated time frame.
  • Maintains a clean, safe and professional department.
  • Presents with a professional appearance and attitude at all times.
  • Adheres to facility dress code if applicable.
  • Maintains regular attendance.
  • Participates in census building and marketing activities, as directed.
  • Participates in clinical program development and implementation, as directed.
  • Carries out responsibilities in compliance with federal, state, local laws and regulations, and with facility philosophy, policies and procedures.
