The Advanced Practice Provider (APP) will be either a Nurse Practitioner or Physician Assistant (NP/PA). The APP will serve a key role on the PM&R consult team in the acute care hospital. The individual will advise primary teams on the management of medical complications of disability (related but not limited to: spinal cord injury, brain injury, limb loss, neuromuscular weakness, musculoskeletal injuries, pain, organ transplant, etc.). The APP will optimize complex hospital discharge planning and post-acute care throughput. The position requires a team player who will partner with a multidisciplinary team (including but not limited to: attending physicians, resident physicians, nurses, physical therapists, occupational therapists, speech language pathologists, case managers, social workers, other consultants, etc.) This role requires a strong foundation in evidence-based practice, with the APP responsible for adhering to established clinical pathways and protocols to ensure safe and effective patient care. The APP will demonstrate advanced medical knowledge, clinical competence, and sound clinical judgment. In addition, the APP is accountable for timely and compliant documentation and actively participates in professional development and competency validation to maintain high standards of care. The ideal candidate will have excellent verbal/ written communication skills and will be a leader who takes initiative to collaborate with primary teams to grow the consult service.
