About The Position

This role involves delivering specialized physiatry care in Skilled Nursing Facilities. The primary focus is on providing care to patients experiencing difficulty participating in therapy due to pain or functional limitations. The physiatrist will perform focused assessments, develop individualized treatment plans in collaboration with facility primary care teams, and provide targeted interventions such as joint injections, Botox treatments, dry needling, and medication management. The role requires a high level of autonomy, operating as the sole physiatrist on-site, with support from a collaborative clinical team for consultation. Care coordination and referrals to other specialties are also key responsibilities. This position is ideal for a self-motivated and confident clinician passionate about improving rehabilitation outcomes in an independent practice environment.

Requirements

  • Current, unrestricted PA or NP license.
  • DEA licensed or willing to obtain DEA licensure.
  • Comfort and interest in performing peripheral joint injections (knees, shoulders, hips).
  • Genuine passion for providing professional, personalized care in an autonomous environment.
  • Strong communication skills, with the ability to collaborate with multidisciplinary teams.
  • Familiarity with EHR charting.

Nice To Haves

  • Nursing home and/or physiatry experience is a plus, but not required.

Responsibilities

  • Round at 2 Skilled Nursing Facilities (Chico, CA and Redding, CA).
  • Deliver specialized physiatry care to 27+ patients per day.
  • Perform focused assessments and develop individualized treatment plans.
  • Provide targeted interventions including joint injections, Botox treatments, dry needling, and medication management.
  • Operate with a high level of autonomy as the sole physiatrist on-site.
  • Coordinate care and refer to other specialties or services.
  • Improve rehabilitation outcomes.
