Requirements
- High school diploma or GED equivalent
- Current License as a Pharmacy Technician in the State of Illinois
- Current Pharmacy Technician Certification or must obtain CPhT (Certified Pharmacy Technician) within 90 days of hire
- Successfully passed the Pharmacy Technician Certification Board (PTCB) or the Exam for the Certification of Pharmacy Technicians (ExCPT) examination pending certification
- Current Pharmacy Technician license with “Student” designation issued by the State of Illinois
Responsibilities
- Assist in the process of collecting Home Medication History of patients being admitted to the hospital.
- Identify recent outpatient medication dispenses using provided resources.
- Interview patients.
- Contact pharmacies.
- Accurately document collected information.
- Work primarily in the Emergency Department, but may also include inpatient floors.
- Work only under the supervision of a registered pharmacist.
