About The Position

The Medication History Pharmacy Technician is an integral member of the Department of Pharmacy. The Medication History Pharmacy Technician is responsible for the medication history process in the Emergency Department and for admitted patients. This individual works independently and cooperatively with members of the Department of Pharmacy, Nursing, and Physicians. This position requires effective communication with these departments to improve patient care. The Medication History Pharmacy Technician provides exceptional customer service as an extension of their patient-oriented duties. This position is responsible for providing an updated medication lists for all high-risk patients. Furthermore, this position demonstrates proficiency in evaluating documented medication histories using structured patient interviews and standardized forms and processes. The Medication History Pharmacy Technician’s role is to improve the accuracy of the histories of medication use prior to admission, and to optimize patient outcomes in the medication reconciliation process. The Medication History Pharmacy Technician has the ability to utilize automation systems, the electronic health record, and various office software programs. The Medication History Pharmacy Technicians will follow and maintain the appropriate documentation as required by law, regulations, and departmental policy.

Requirements

  • Minimum three (3) year of experience in an inpatient hospital pharmacy or community pharmacy setting required
  • Current California Registered Pharmacy Technician license required

Nice To Haves

  • Graduated from a Pharmacy Technician Training Program preferred
  • Current Certified Pharmacy Technician (CPhT) certificate preferred

Responsibilities

  • Performs thorough patient medication histories and document correctly into the electronic medical record.
  • Ability to performs the duties and functions of a dispensing Pharmacy Technician.
  • Demonstrates excellent customer service to all patients and visitors.
  • Participates in quality assurance programs by maintaining medication history data and reporting any significant medication errors.
  • Ability to be at work and be on time.
  • Ability to follow company policies, procedures and directives.
  • Ability to interact in a positive and constructive manner.
  • Ability to prioritize and multitask.
  • Demonstrates exceptional customer services skills with patients and colleagues as well as communicates effectively with other departments
  • Displays attention to detail when documenting prior to admission medication history.
  • Educates oneself and trains/ educates students and new staff
  • Demonstrates leadership skills that positively contribute to the Medical Center
  • Provides pharmaceutical care in accordance with law and regulations
  • Supports the direction and initiatives of the Department of Pharmacy
