About The Position

Under the leadership of the Director of Pharmacy, the Medication Reconciliation Technician is an active member of the Inpatient Pharmacy team that assists pharmacists in obtaining lists of the most accurate patient medication histories prior to admission by interviewing patients and consulting with their primary care provider, specialists, and preferred community pharmacies. The Medication Reconciliation Technician will obtain medication histories in the Emergency Department as well as in the inpatient areas for all patients that are being admitted to the hospital. Through performing medication histories, the Medication Reconciliation Technician plays an integral role in helping to prevent medication errors caused by inaccurate or incomplete information. The technician will be expected to provide exemplary service to other internal customers at Crisp Regional Hospital, as well as embody the organizations values in all interactions with patients and colleagues. This position will rotate through days, evenings, weekends, and holidays.

Requirements

  • Requires high school diploma or GED.
  • Requires a minimum of five years of work-related on-the-job experience or any equivalent combination of education, training and experience.
  • Requires active registration as a pharmacy technician with the Georgia Board of Pharmacy, as well as national certification as a pharmacy technician through the Pharmacy Technician Certification Board (PTCB).

Nice To Haves

  • PTCB Medication History Certificate preferred upon hire and required within 12 months of hire.

Responsibilities

  • Obtains medication histories on all assigned patients during scheduled shifts
  • Ensures that prescriptions be sent to Crisp Regional Retail Pharmacy when appropriate
  • Performs patient and/or caregiver interviews to obtain dose and frequency of current medications including prescriptions, over-the-counter medications, and herbal supplements
  • Calls community pharmacies or other healthcare facilities to confirm medication regimens or investigate discrepancies when necessary
  • Communicates medication-related issues to all appropriate health care professionals
  • Writes formal notes in the electronic medical record, where appropriate, documenting the findings of the medication history
  • Provides exemplary customer service to Crisp Regional Hospital staff and patients
  • Delivers exceptional care to patients with empathy and respect
  • Maintains the confidentiality of all designated hospital and patient information at all times
  • Conforms to Crisp Regional Hospital’s standards of conduct so that the best possible customer service and patient care experience is provided:
