About The Position

Under the supervision of a licensed pharmacist or a licensed healthcare professional (i.e. physician, nurse practitioner or nurse), the Medication Reconciliation Pharmacy Technician's primary responsibility is to perform medication history assessments for new patients coming through the Emergency Department or new inpatient direct admissions to the hospital. Medication history information will be obtained from patients directly or the patient's family member/caregiver, retail pharmacy or physician office. The Medication Reconciliation Pharmacy Technician will enter medication histories into the electronic database or will update the database if the patient has existing medication information--this involves compiling and verifying patient medication lists, comparing those lists with current prescriptions, and communicating any discrepancies or questions to the patient's nurse, nurse practitioner and/or other healthcare providers. They will accurately complete information regarding each medication identified (i.e., drug, dose, route, frequency, indication for PRN, and last dose taken-date/time); verify patient allergy information and ensure complete documentation (i.e., substance, allergy category, reaction description). Communication of the completed medication history will be given to the physician, practitioner and/or nursing team in a timely manner.

Requirements

  • High School Diploma or GED required
  • Alabama Pharmacy Technician Registration Required
  • Pharmacy Technician Certification (PTCB) or C-NHA-Certified Pharmacy Technician required to be completed within 6 months of hire.
  • Possess the necessary interpersonal and communication skills required to interact with patients, coworkers and other health professionals
  • Ability to prioritize work in a high volume work environment
  • Ability to write legibly, speak, and read English proficiently
  • Ability to operate computer, calculator, telephone
  • Adaptable to changes in workload and schedule to meet patient needs
  • Ability to set priorities and solve problems

Nice To Haves

  • Successful completion of a certified pharmacy technician training program is preferred.
  • Working knowledge of common outpatient medication doses and forms preferred.
  • Three or more years of pharmacy technician experience in a retail setting preferred
