About The Position

Wilmington Health is seeking a full-time Value Based Care Pharmacy Technician to systematically close care gaps and address patient-specific barriers to adherence. Requirements

  • Certified Pharmacy Technician
  • Nexgen-EHR experience
  • 3+ years of pharmacy retail experience
  • Ability to work independently and take initiative in addressing concerns and issues.

Nice To Haves

  • Established fluency in NextGen allows the pharmacy technician to navigate charts and send refills without a 3–6 month learning curve.
  • Understanding of clinical workflows, refill protocol requirements, provider preferences, and internal communication protocols.
  • A CPhT with 3+ years of experience in a retail setting is preferred, with hands-on experience resolving barriers (e.g. adjudication, out-of-stock, preferred manufacturers).

Responsibilities

  • Proactively manage adherence reports for MAC, MAH, and MAD. Identify reasons for non-compliance (e.g., "silent" barriers like overdue appointments, lab requirements, expired refills, or clinical contraindications).
  • Coordinate the refill process by identifying patients who have not picked up medications. Facilitate "bridge" supplies, update discontinued orders, and ensure patients have visits and labs scheduled to maintain therapy.
  • Address cost barriers by referring patients to our Patient Assistance Department, suggesting therapeutic generic alternatives, or navigating insurance coverage issues using our Prior Authorization Department.
  • Partner with the on-call Clinical Pharmacist or provider as necessary for complex cases involving side effects, allergies, or therapy optimizations.
  • Analyze monthly data to identify company-wide "patterns that increase the risk for non-adherence." Report findings to the Department Manager and assist in developing education or reporting tools to address these long-term barriers.
