Requirements

Graduated from an ACPE accredited College of Pharmacy (required)
Post-Graduate Year 1 (PGY-1) Pharmacy Residency required or three (3) years of relevant work experience (required)
Licensed or license eligible as a pharmacist in the Commonwealth of Kentucky (required)
Current certification in Basic Life Support or must obtain within 90 days from date of hire (required)

Nice To Haves

Two (2) years of leadership experience (preferred)
Current board certification by Board of Pharmacy Specialties® or must obtain within two (2) years from date of hire (preferred)

Responsibilities

Manage and lead the provision of safe, timely, efficient, equitable, effective, and patient-centered pharmacy patient care services and operations for areas of responsibility
Responsible for operational systems to assure appropriate prescribing, distribution, administration, and monitoring outcomes of drug therapy process
Assure that existing automation is being utilized at the highest level of efficiency
Assure compliance within sterile compounding (USP 797), non-sterile compounding practices (USP 795), and handling of hazardous drugs (USP 800)
Assist and coordinate efforts to ensure operations meet compliance and quality improvement standards and initiatives
Maintain quality assurance programs for pharmacy services designed to objectively and systematically monitor care, pursue opportunities for improvement, resolve identified problems as may exist, and detect and prevent drug diversion
Oversight of records retention per hospital policy, including but not limited to: employee records, controlled substance invoices, compounding records, etc.
Identify opportunities to redesign operational systems to maximize labor and operational efficiency
Provide oversight of day-to-day operations while identifying and resolving problems which occur
In collaboration with Medication Safety Manager, review reports for medication errors, adverse drug events, ineffective service, or other problem identification. Analyze and implement quality improvement measures for improving organizational performance
Assure proper training for end users
Assures all pharmacy permits, registrations, staff licenses and certifications are current
File reports required by state or federal laws and regulations
Contribute to, implement and maintain pharmacy policies and procedures related to operations including, but not limited to: procurement, storage, security, and disposition of drugs and provision of pharmacy services
Stabilize and continually monitor core processes of drug distribution and medication safety. Identify opportunities to improve processes based on feedback from staff and coordinate implementation of improvements in a timely manner
Remove barriers for staff to ensure optimal performance
Collaborate with other departments or disciplines to improve medication use safety and efficiency
Works diligently to eliminate waste through creative solutions and staff engagement
Work collaboratively with other clinicians and health-care providers during routine communications
Review medication orders for accuracy, appropriateness and efficacy
Discuss medication order clarifications with prescriber, update orders to reflect communicated changes, and notify involved personnel
Document all pharmacy interventions in patient electronic medical record and integrated clinical decision support system
Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
Promote and demonstrate rational, cost-effective drug therapy
Works to promote compliance with established policies within the department of Pharmacy and the Pharmacy & Therapeutics Committee.
Participate in formulary management by implementing therapeutic interchanges and enforcing the non-formulary ordering process
Facilitate the implementation of clinical and operational initiatives
Participates or provides oversight of participation in hospital standing and ad-hoc groups concerned with medication utilization and safety to define present and future needs and to assist in problem resolution
Provides monthly and/or periodic reports to administration containing qualitative and/or quantitative data
Facilitate and participate in scholarly and professional development activities
Provide guidance and supervision to pharmacists and certified pharmacy technicians
Precept pharmacy students during IPPE and APPE rotations as necessary
Supports drug research conducted by the institution. In collaboration with the pharmacy institutional review board (IRB) representative, helps to ensure optimal handling of investigational agents and appropriate documentation
Active participation in local, regional, or national organizations in area of expertise encouraged
Collaborate with pharmacy leadership in developing education, training, and competency programs
Execute hospital and pharmacy policies and procedures
Collaborate with pharmacy leadership in development and review of departmental policies and procedures
Provides leadership to ensure successful compliance with all federal, state and local regulatory standards.
Function as a role model and as an educational resource for pharmacy staff
Oversees the recruitment, orientation, and evaluation of quality staff
Serves as a mentor to pharmacy staff, aiding in the development of their clinical skills.
Utilize advanced communication skills to convey and demonstrate clinical pharmacist expertise
Form collaborative relationships with pharmacy team members and other members of the healthcare team
Demonstrate effective professional and patient communications including articulation of recommendations
Demonstrate the ability to work in team environment, reaffirming the contribution of each person to the delivery of care/services
Achieve and maintain satisfactory working relationships with pharmacy leadership, pharmacy staff and other departments throughout the system, including committees and administration
Attend and lead departmental staff huddles
Maintains compliance with all company policies, procedures and standards of conduct
Complies with HIPAA privacy and security requirements to maintain confidentiality at all times
Performs other duties as assigned
