About The Position

Texas Oncology is seeking a Pharmacy Resident to join their team at the Texas Oncology Corporate location in Dallas, Texas. As the largest community oncology provider in the country, Texas Oncology offers a mission-driven environment focused on providing high-quality, evidence-based cancer care. The Pharmacy Resident will work under the supervision of the Residency Director, compounding and dispensing medications, maintaining records, and consulting with healthcare professionals and patients. This role is crucial in ensuring the safe and effective use of medications within the organization, adhering to professional standards, legal regulations, and quality assurance policies.

Requirements

The applicant must be a graduate of an ACPE-accredited Doctor of Pharmacy (PharmD) program.
Must be a citizen of the USA or hold a visa allowing for the completion of the residency year.
Must obtain licensure to practice pharmacy In the state of Texas within 8 weeks of the residency start date.
May supervise the activities of pharmacy technicians.

Responsibilities

Perform the following functions as a licensed, practicing pharmacist:
Prepares and dispenses medication orders. Maintains and evaluates patient profile and comprehensive drug data file to evaluate orders for any incompatibilities.
Checks all orders for completeness of information. Verifies patient information and proper physician authorization. Checks all orders for insurance approval before admixing.
Admixes drugs as ordered by physician in an accurate and timely manner as per department guidelines, and preparing all drugs within OSHA guidelines and interoffice guidelines.
Ensures correct preparation of all formulas or Checks all prescriptions for completeness. Calculates correct dosages, converting between metric and apothecary equivalents.
Organizes and directs technician's workflow; verifying their preparation and labeling of pharmaceuticals; verifying order entries, charges, and inspections. Provides pharmacological information by answering questions and requests of health care professionals and patients.
Provides and documents drug counseling to all patients. Ensures all legal and professional requirements are adhered to, including controlled substances. Consults with patients to determine pre-existing conditions to assess medication.
Instructs patients on effects of food and alcohol intake. Warns of potential side effects. Provides professional drug information and assistance to medical staff.
Participates in pharmacy teaching and in-service programs. Properly disposes all materials used when mixing chemo or non-chemo IVs. Cleans hood daily and in between V. admixturing.
Logs and dates all drugs that are being used. Checks drugs monthly for outdates. Keeps accurate temperature logs.
Keeps accurate logs on all protocol drugs and patient assistance drugs. Assists with inventory control of medications. Orders supplies and Receives and shelves medications.
Ensures that standard drug supplies are in adequate supply in all departments. Prepares labels. Files and prepares Computes prices.
Receives new drug education from drug representatives; remains abreast of new Maintains confidentiality of all information related to patients and medical staff.
Prepares reports and monitoring activities as directed.
Check all orders for completeness of information.
Verify pathway status and document status in the EMR. If regimen is off pathway, ensure a pathway exception has been approved by the regional medical director prior to treatment.
Verify compendia compliance and requests off compendia forms as applicable.
Verify appropriate dosing and schedule of all medications.
Round dosing per TxO policy.
Verify appropriate premedications are ordered. Add additional premedications as directed by TxO policy. Adjust antiemetics per the antiemetic protocol and notify physician of any changes warranted.
Review most current labs and notify appropriate site personnel if needed labs are missing or dose adjustments are needed.
Verify a treatment plan is documented in the EMR and notify the ordering physician if the treatment plan is missing.
Review the insurance alert for commonly confused jcodes to ensure the correct drug is authorized.
Notify business office personnel that the medication or regimen has been reviewed by pharmacy.
Review oral chemotherapy agents or antiemetics for any drug interactions with current list of medications—if found alert prescriber/pharmacist/or APP. Make recommendations for alterative medication as needed or dosing modifications. Check that an oral chemotherapy prescription has been sent to pharmacy with appropriate instructions.
Verify medications adhere to the therapeutic interchange policy. Contact the physician for needed interchanges. If the interchange is denied, the reasoning is to be documented in the pharmacy note.
Follow TxO No Mix policy and other guidelines as directed by pharmacy administration.
Verify the appropriate primary diagnosis code is used for regimen entry. Also verify secondary and tertiary codes when appropriate.
Research drug information or other clinical needs as requested by physicians.
Prepares reports and monitoring activities as directed.
Document all activities and pending issues in the pharmacy note in the EMR.
In addition to the above, the position will be responsible for the following:
Travel to various Texas Oncology regions
Presentation of an abstract publication at an oncology conference
Provide quarterly virtual educational sessions for TXO Staff
Provide at least one continuing education presentation during the residency year
Present medication and business-releated updates to physician leadership
Attendance at monthly Pharmacy and Therapeutics (P&T) meetings
Prompt completion of residency-required learning evaluations
Successfully design, carry out, and develop a manuscript of a major project to be determined within the first 2 months of the residency start date. The objective is to present a poster at the NCODA Spring Forum Clinical Meeting. A manuscript will be developed and completed by the end of the residency year, and publication is strongly encouraged, though not required.
