About The Position

A Pharmacy Resident possesses the skills and ability to assess data reflective of patient status and interpret this information and identify the patients' needs in relation to age specific requirements. Provide care as described in accordance with state and federal law and regulation, and facility policy and procedures. May be designated by the Director of Pharmacy, or designee, as being in charge. Supervises and performs compounding (both sterile and non-sterile), packaging and dispensing of pharmaceuticals according to prescription, physician order or floor stock request and monitors patient profiles to assure appropriate dosage and therapy.

Requirements

  • Must have a Doctor of Pharmacy degree or be a candidate for graduation from a college or school accredited by the Accreditation Council for Pharmacy Education (ACPE) (Required)
  • If a foreign graduate of pharmacy, must have completed all requirements for equivalency as outlined by the National Association of Board of Pharmacy (NABP) including Foreign Pharmacy Graduate Examination Committee (FPGEC) Certification (Required)
  • Licensed Pharmacist - Virginia Department of Health Professions (VDHP) within 90 Days(Required)

Nice To Haves

  • Prior hospital pharmacy experience
  • Licensed Pharmacist - Virginia Department of Health Professions (VDHP) within 60 Days(Preferred)

Responsibilities

  • Supervise and perform compounding (both sterile and non-sterile), packaging, and dispensing of pharmaceuticals according to prescription, physician order, or floor stock request in compliance with state and federal law and regulation
  • Review, validate, and assess patient specific medication orders.
  • Monitor patient profiles to assure appropriate dosage and therapy.
  • Responsible for all core clinical pharmacy programs and commitment to evidence-based patient care, to including, but not limited to IV to PO, renal/hepatic dosing, therapeutic interchange, antimicrobial stewardship, anticoagulation management, duplicate therapy, drug-drug interactions, non-formulary management and weight based dosing
  • Handle all aspects of pharmacy operations to include order review and validation, sterile and non-sterile compounding, controlled substance/diversion management, pharmacy automation, and all state/federal law and regulation
  • Establish an environment of safety and ensure safe practices for the patient as evidenced by recognizing and implementing safe medication practices, and following policies and procedures that comply with regulatory agency regulations
