About The Position

The Pharmacy Resident PGY2 (postgraduate year two) is a healthcare professional who has completed their Doctor of Pharmacy (PharmD) degree and postgraduate year one (PGY1) pharmacy residency program. This resident is undergoing specialized training in a pharmacy residency program. The position works under the close supervision of experienced pharmacists to gain practical experience in various aspects of pharmacy practice. The role is responsible for providing advanced pharmaceutical care to patients, including medication therapy management, drug information, and patient counseling. The Pharmacy Resident PGY2 also participates in clinical rounds, research projects, and educational activities. The position plays a crucial role in optimizing patient outcomes and developing their clinical skills to become competent and confident specialized pharmacists.

Requirements

• Doctor of Pharmacy degree and completion of a Postgraduate Year One Pharmacy Residency Program
• BLS (CPR)-American Red Cross or AHA Healthcare Provider.
• Certificate of PGY1 program completion required within 30 days of hire.
• Registered Pharmacist in NC within 60 days of hire.

Responsibilities

• Assesses patient medication profiles and medical histories to identify potential drug interactions, allergies, and other medication-related issues.
• Collaborates with healthcare teams to develop and implement patient-specific medication therapy plans, ensuring safe and effective drug therapy.
• Educates patients and healthcare professionals on medication use, including proper administration, potential side effects, and drug interactions.
• Conducts research projects to contribute to the advancement of pharmacy practice and improve patient care outcomes.
• Monitors patients' responses to medication therapy, including therapeutic outcomes and adverse effects, and makes appropriate recommendations for adjustments.
• Accurately documents patient medication histories, interventions, and outcomes to ensure continuity of care and facilitate effective communication among healthcare providers.
• Performs other duties as assigned.
