Cedars-Sinai Medical Center is seeking a clinical and strategic pharmacy leader to advance a comprehensive diabetes stewardship program. This role involves working collaboratively with physicians, pharmacists, nurses, diabetes educators, informatics specialists, and other members of the interdisciplinary diabetes team to reduce patient harm related to insulin administration, optimize institutional glucometric outcomes, and promote evidence-based pharmacotherapy for hospitalized patients with diabetes. The position will involve designing and implementing a real-time surveillance tool within the electronic health record to identify patients with glycemic excursions, support timely care triage, and enhance the delivery of individualized pharmacotherapeutic recommendations. The role also includes providing clinical support to frontline pharmacy colleagues through medication reviews, therapy optimization, and management of complex inpatient diabetes cases. Additionally, the coordinator will help with medication use evaluations, quality improvement initiatives, clinical audits, and outcomes reporting to improve glycemic management, medication safety, and patient care. This role aims to establish consistent, measurable, and evidence-based diabetes care across Cedars-Sinai, contributing to patient safety, clinical excellence, education, and innovation.
