The Pharmacy Clinical Specialist – Internal Medicine provides advanced clinical pharmacy services at Hillcrest Hospital, supporting hospitalist medicine teams and interdisciplinary care teams across multiple inpatient units. This caregiver optimizes medication therapy for complex hospitalized patients through clinical rounds, medication management, evidence-based recommendations, and collaboration with physicians and healthcare providers. Additional responsibilities include educating pharmacy residents and learners, participating in medication safety and stewardship initiatives, contributing to research and quality improvement projects, and supporting clinical protocol development. This role includes staffing responsibilities approximately every ninth weekend. A caregiver in this position will work Monday through Friday from 7:00am to 3:30pm with staffing requirements for weekends occurring every 9th weekend. No holidays or on-call expectations required.

Requirements

  • PharmD or Bachelor’s Degree in Pharmacy from an accredited college of Pharmacy. If hired for a specialty area where specialty residency training is available, then specialty residency is required
  • Current state licensure as a Registered Pharmacist (RPh) or eligible within 90 days of hire
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire (if administering immunizations)
  • Completion of a Post-Graduate Year 2 Residency (PGY2) or fellowship OR completion of a Post-Graduate Year 1 Residency (PGY-1) and two years of experience as a Clinical Specialist or an equivalent role, including providing pharmaceutical care and performing pharmacotherapeutic monitoring for at least 50% of time in the specialty area OR four years of experience as a Clinical Specialist or an equivalent role, including providing pharmaceutical care and performing pharmacotherapeutic monitoring for at least 50% of time in the specialty area

  • Experience participating in clinical rounding within hospitalist or internal medicine team

Responsibilities

  • Routinely and continually provide pharmaceutical care for patients, considering their age and clinical condition.
  • Provide general and patient specific drug information to patients, physicians, nurses and, and other health care professionals.
  • Support Clinical Pharmacists in providing pharmaceutical care to patients by direct interaction and formal educational programs.
  • Maintain expertise in areas of practice through continual learning, journal review and other educational activities.
  • Educate Clinical Pharmacists, Technicians, Pharmacy Students, Post-Graduate Trainees and other caregivers in Pharmacotherapy within in the specialty area.
