About The Position

The Pharmacy Community Health Worker (CHW) plays a vital role in improving patient access to medications and related health resources across the health center. This position supports patients with medication access, affordability, and adherence; assists with Medicare and prescription assistance navigation; and coordinates prescription services between facilities. The Pharmacy CHW collaborates closely with the pharmacy team, clinical staff, and community partners to address social drivers of health (SDoH), reduce barriers to care, and improve health outcomes.

  • No prior experience required; skills may be learned on the job
  • Ability to communicate effectively with patients from diverse backgrounds
  • Strong organizational skills and attention to detail
  • Willingness to complete required training and cross-training
  • Ability to maintain patient confidentiality and comply with HIPAA requirements
  • Comfort working in a fast-paced healthcare environment
  • Reliable transportation for community outreach activities, if applicable

Nice To Haves

  • Previous experience as a Community Health Worker
  • Previous experience as a Pharmacy Technician
  • Experience working in a healthcare, community-based, or patient support role

Responsibilities

  • Crosstrain to perform Pharmacy Technician support functions as needed
  • Assist patients with medication access, adherence, and medication affordability options
  • Guide patients through Medicare Part D enrollment, plan optimization, and prescription assistance programs
  • Support initiatives to increase utilization of Four Rivers Pharmacy services
  • Coordinate prescription delivery and transfer processes across health center locations
  • Screen patients for SDoH needs including insurance coverage, food insecurity, housing stability, employment, transportation, and access to healthcare
  • Connect patients to appropriate internal services and community-based resources
  • Document identified needs, referrals provided, and follow-up actions in the medical record when applicable
  • Conduct outreach to patients with prescriptions pending pick-up for more than 12 days
  • Identify and document barriers to medication access or adherence
  • Collaborate with pharmacy staff and care teams to resolve barriers and develop patient-centered solutions
  • Support initiatives aimed at improving medication adherence and continuity of care
  • Participate in community events to promote pharmacy services and medication-related education
  • Provide patient education on medication management, chronic disease support, preventive care, and healthy behaviors
  • Serve as a liaison between patients, the pharmacy team, and community partners
