About The Position
The Pharmacist role at Atlantic Health Morristown Medical Center is a per-diem position focused on ensuring optimal patient therapy through accurate medication profiling, dispensing, and clinical interventions. This role involves reviewing patient-specific information to tailor medication plans, accurately transcribing and dispensing medications, checking unit doses and sterile preparations, and providing drug information to healthcare professionals and patients. Pharmacists are integrated into care teams to ensure safe and effective medication use.
Requirements
- Valid Pharmacist license in the state of New Jersey.
- Experience in reviewing patient profiles for appropriate therapy.
- Proficiency in transcribing and dispensing medications accurately.
- Experience with sterile compounding, including hazardous drugs.
- Knowledge of IV equipment, sterile technique, and total parenteral nutrition.
- Ability to provide drug information to healthcare professionals and patients.
- Familiarity with electronic health records (EHR).
Nice To Haves
- Experience with antineoplastic agents.
- Knowledge of Pharmacy and Therapeutics Committee initiatives.
- Experience converting non-formulary medications to formulary alternatives.
Responsibilities
- Reviews patient profiles to assure appropriate therapy based on patient-specific information including age, gender, diagnosis, co-morbidities, laboratory values, allergies, height, weight, BSA, and pregnancy/lactation status.
- Reviews and transcribes all medication orders accurately and timely, verifying therapy, medication, route, dose, and frequency.
- Makes recommendations for alternate treatments when necessary.
- Records relevant information and drug waste in EHR.
- Dispenses medications, prescriptions, and miscellaneous items accurately and promptly.
- Processes requests via phone, window, fax, and computer professionally and timely.
- Obtains medications considering economical or alternative options and patient admission status.
- Checks unit dose, medications for automated dispensing machines and floor stock, IVs, batching, patient own medications, repackaging, extemporaneous preparations, crash cart trays, rapid response boxes, and emergency kits for accuracy.
- Prepares, supervises, and ensures aseptic compounding of IVs, including antineoplastic agents, promoting efficiency and cost-effectiveness.
- Reviews levels of batched IV medications, batches, and supplies to avoid waste.
- Assures batches are checked and put away promptly.
- Participates in clinical initiatives and initiates appropriate clinical interventions and clarifications with corresponding documentation.
- Reviews pertinent information to monitor patients’ therapies and makes appropriate recommendations to eliminate duplicate, unwarranted, or inappropriate therapy.
- Converts non-formulary medication requests to appropriate formulary alternatives whenever possible.
- Provides drug information to physicians, nurses, other healthcare professionals, and patients/family members.
- Demonstrates and shares knowledge of various drug information resources and research methods.
