Requirements

  • Must be a graduate of an accredited school of pharmacy.
  • BS Pharm or Pharm D degree required.
  • Must hold a current state license to practice pharmacy.
  • Excellent oral and written communication and interpersonal skills.

Nice To Haves

  • Accredited clinical residency preferred.
  • Two years’ experience in hospital pharmacy is preferred.

Responsibilities

  • Dispensing of prescription medications to hospital inpatients.
  • Providing information about medications and helping patients and clinical staff understand instructions provided by the physician.
  • Monitoring the patient’s health and progress to assure medications are effective and safe.
  • Adhering to state and federal regulations as well as Joint Commission standards.
  • Verifying physician medication orders with accuracy and timeliness.
  • Monitoring and evaluating the six critical processes of medication management; determining risk points and being an active participant with physicians and clinical staff in continuous quality and performance improvement.
  • Verifying and approving the work of Pharmacy Technician(s).
  • Responsible for the management and utilization of the computerized pharmacy system.
  • Ordering and storing medications, keeping them safe and effective.
  • Monitoring drug therapy for appropriateness and promoting/evaluating rational drug therapy that improves pharmaceutical care.
  • Performing anticoagulant monitoring.
