About The Position

The Pharmacist will be responsible for having a working knowledge of all areas of pharmaceutical services provided by the hospital, including drug distribution systems, central operations, and IV admixture. This role involves performing all dispensing related functions, including entering medication orders, preparing, labeling, and dispensing medications, compounding pharmaceutical preparations using aseptic technique, and filling medication cassettes. The Pharmacist will also prepare weekend and day pass medications, confer with physicians regarding medication orders, document recommendations, and perform antibiotic stewardship activities. Additionally, the role includes participating in clinical activities such as attending Team Conferences, patient rounds, medication management teaching, mentoring pharmacy students, processing drug information inquiries, and consulting with patients and families on discharge medications. The Pharmacist will also serve as a resource to other departments and consult with medical and nursing staff on drug information.

Requirements

  • Current state licensure as a Pharmacist required

Responsibilities

  • Perform all dispensing related functions, including entering medication orders into the computer generated patient profile.
  • Prepare, label, and dispense individual medications in accordance with doctor's prescriptions.
  • Compound oral and injectable pharmaceutical preparations using proper aseptic technique.
  • Fill and check medication cassettes to ensure accuracy prior to delivery to the patient care unit.
  • Prepare weekend and day pass medications for patients on leave from the hospital.
  • Contact and confer with physicians regarding any questions or irregularities regarding medication orders.
  • Document all recommendations and corrective actions taken in a clinical intervention report.
  • Perform antibiotic stewardship activities by reviewing pertinent culture and sensitivity information as well as laboratory data and making therapeutic recommendations.
  • Participate in clinical activities including attending Team Conferences, participating in patient related rounds, conducting medication management teaching, mentoring pharmacy students during their rotations, processing drug information inquiries and consulting patients and family members on discharge medications.
  • Keep informed on developments and trends inside and outside the organization which relate to assigned functions.
  • Serve as a resource to other departments for information needed.
  • Participate in and contribute to outside professional activities or other projects.
  • Consult with physician and nursing staff concerning availability, form, side effects and incompatibilities of currently available drugs.
  • Maintain drug literature source to be at disposal of medical and other professional staff.
