About The Position

Ashley Regional Medical Center, part of Lifepoint Health, is seeking a full-time Pharmacist to coordinate and provide pharmaceutical care for patients. The role reviews and verifies medication orders, manages drug therapy protocols, provides education, and supports quality improvement and regulatory standards. The Pharmacist also manages patients in an outpatient anticoagulation clinic, including point-of-care testing, evaluation of results, dosing, and scheduling.

Requirements

• Doctorate degree in pharmacy.
• Licensed Pharmacist in Utah.
• Basic Life Support certification.

Nice To Haves

• ACLS certification.

Responsibilities

• Manage drug therapy protocols as part of an interdisciplinary team.
• Review, interpret, and evaluate medication orders to identify, prevent, and resolve drug-related problems.
• Provide drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
• Document interventions and outcomes for drug therapy quality improvement.
• Verify medication orders for accuracy and completeness before entry; check filled medications prepared by technicians; and maintain proficiency with pharmacy automated systems and software.
• Manage patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
