Requirements

Bachelors Degree
1-2 years of previous job-related experience
Registered Pharmacist in the State of Illinois through the IDFPR website
ACLS (Advanced Cardio Life Support Certified)
CPR (Cardiopulmonary Resuscitation)

Nice To Haves

Doctor of Pharmacy (PharmD) preferred
Residency: Pharmacy Practice - General (Preferred)
Residency - Specialty(Preferred)
Board Certified in Specialty Area

Responsibilities

Provide comprehensive pharmaceutical care to designated service areas or patient populations to achieve optimal patient outcomes and safety.
Function as a member of multidisciplinary teams to improve medication therapy management.
Act as drug information resources.
Perform age-specific and/or disease specific consultative medication therapy evaluation, education, research and participation in quality improvement activities.
