The staff pharmacist position coordinates the drug distribution and clinical pharmacy activities within the hospital for nursing and support departments. This position is responsible for safe and efficient methods of coordination of pharmacy policy concerning patient drug care and also provides clinical drug information which meets the needs of the patient, nurse, and physician.

Requirements

  • Graduate from an accredited College of Pharmacy
  • Licensed or eligible for licensure in the State of Minnesota
  • Ability to work rotating shifts and holidays

Responsibilities

  • Review and interpret physician orders for appropriateness
  • Dispense prescriptions to patients in accordance with hospital policy and state and federal laws
  • Maintain an up to date patient profile ensuring an accurate eMAR
  • Review drug orders for incompatibilities, interactions, contraindications, and allergies
  • Verify the work performed by the technician staff including: data entry, cassette filling, unit dose drug distribution, and IV compounding
  • Supervise the pre-packing and required recordkeeping of pharmaceutical products
  • Review controlled drug delivery requests by the pharmacy technician to ensure accuracy and proper record keeping
  • Prepare IV solutions for administration via the intravenous route and ensure the sterility/stability of these solutions
  • Compound chemotherapy drugs for the oncology infusion center
  • Double check the work of the compounding pharmacist or technician
  • Verify the dose, administration parameters, and indication for all oncology regimens
  • Review chemotherapy drugs including dose, administration, indication and safety
  • Participate in clinical programs within the department such as: drug level monitoring, antibiotic monitoring and pharmacokinetic consults
  • Complete admission medication reconciliation for hospital inpatients on an as needed basis
  • Maintain up to date knowledge of the profession by participating in organizational memberships: meetings, lectures, in-services, or seminars
  • Provide drug information (dosing, compatibilities, and formulary policies) to physicians, nurses, and other health care practitioners
  • Respond to emergencies (i.e. Code Blue) within facility
