About The Position
Bruyère Health is seeking a Pharmacist for a temporary, full-time position. This role involves conducting individual patient assessments, providing recommendations and follow-ups on medication usage, educating patients on medication use, and collaborating with the interprofessional team to enhance the medication management program for improved patient care. The position reports to the Director, Pharmacy and is under the supervision of Pharmacy Leadership.
Requirements
- Undergraduate degree in Pharmacy
- Minimum of one (1) year experience as a Pharmacist in a patient oriented environment
- Registration as Pharmacist with the Ontario College of Pharmacists
- Hospital pharmacy residency or equivalent clinical experience

Responsibilities
