About The Position

The Pharmacist coordinates and provides pharmaceutical care for patients, collaborating with the interprofessional team to optimize safe and effective medication use. The role reviews and verifies medication orders, manages drug therapy protocols, educates staff and patients, and supports quality and regulatory standards. The pharmacist also manages patients in an outpatient anticoagulation clinic, including point-of-care testing, evaluating results, dosing, and scheduling. This position reports to the Director or Manager of the Department.

Requirements

• Doctor of Pharmacy degree.
• Current license with the state Board of Pharmacy.
• BCLS certification.

Responsibilities

• Manage drug therapy protocols as part of an interdisciplinary team.
• Review, interpret, and evaluate medication orders to identify, prevent, and resolve drug-related problems.
• Provide drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
• Document interventions and outcomes for drug therapy quality improvement.
• Verify medication orders for accuracy and completeness before entry.
• Check medications filled by technicians.
• Maintain proficiency with pharmacy automated systems and software.
• Manage patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.
• Participate in performance improvement and required continuing education.
• Perform other duties as assigned.
