Providing clinical support of the hospital pharmacy function in a fast-paced environment. Correctly informing and instructing patients on any drugs to be used after they are discharged. Developing and participating in new employee training and education programs. Participating in patient drug therapy monitoring. Promoting an environment of continuous quality improvement.
Requirements
- SC State Pharmacist license in good standing
- Knowledge of general hospital pharmacy practice and clinical pharmacy practice
- Ability to work well in a team setting
- EPIC experience
- Communication skills
- Clinical knowledge
Responsibilities
- Providing clinical support of the hospital pharmacy function in a fast-paced environment.
- Correctly informing and instructing patients on any drugs to be used after they are discharged.
- Developing and participating in new employee training and education programs.
- Participating in patient drug therapy monitoring.
- Promoting an environment of continuous quality improvement.
- Gathering prior to admission home medication list from patient, family, or pharmacy.
- Discharge counseling with patients/families.
- Antimicrobial Stewardship (Interpretation of culture results for recommendations, Knowledge of UTI, pneumonia, SSTI, etc. for recommendations).
- Pharmacokinetic Dosing Management/Monitoring.
- Anticoagulation Dosing Management/Monitoring.
