About The Position

HSHS St. Joseph's Hospital in Breese is seeking a Pharmacist to join their team. This role acts as a liaison between allied health professionals and registered nurses, ensuring accurate and rational drug therapy for patients by reviewing orders for dosing accuracy, drug selection, interactions, allergies, and appropriateness. The Pharmacist also serves as a medical team consultant, participating in multidisciplinary rounds, committees, and workgroups, and provides drug information to allied health professionals, nurses, patients, and the general public. Requirements

  • Meets educational requirements established in the state of practice
  • Graduate of accredited Pharmacy program
  • Eligible for Pharmacist licensure in state of practice
  • Licensed Pharmacist in the state of practice or is eligible to apply for pharmacy board exam/state of practice licensure is required
  • If not currently a Licensed Pharmacist in state of practice, must obtain within 180 days of hire
  • COVID-19 vaccines or an approved medical or religious exemption for all colleagues (does not apply to positions that are classified as 100% remote)

Responsibilities

  • Acts as liaison between allied health professionals and registered nurses in providing accurate and rational drug therapy to patients upon receipt of an allied health professional's order.
  • Reviews order for dosing accuracy, drug selection, drug interactions, allergies, and appropriateness.
  • Serves as a medical team consultant by participating in multidisciplinary rounds, committees, and workgroups.
  • Provides drug information to allied health professionals, nurses, patients, and the general public.
