About The Position

This role involves providing comprehensive pharmacy services within a healthcare setting. The Pharmacist will be responsible for reviewing and verifying medication orders, offering clinical support to healthcare professionals, monitoring patient medication therapy, counseling patients and staff on medication use, participating in medication reconciliation and patient care planning, and maintaining accurate pharmacy records.

Requirements

• Licensed in the State of Iowa as a Registered Pharmacist
• Graduate of an accredited college of pharmacy
• 2–4 years of clinical pharmacy experience in a hospital setting
• Strong knowledge of medication management, pharmacotherapy, and patient safety.
• Strong written and verbal communication skills.

Responsibilities

• Review and verify medication orders for accuracy, appropriateness, and potential drug interactions.
• Provide clinical pharmacy support to physicians, nurses, and other healthcare professionals.
• Monitor medication therapy and recommend adjustments when appropriate.
• Counsel patients and healthcare staff regarding medications, dosage, administration, and potential side effects.
• Participate in medication reconciliation and patient-care planning.
• Maintain accurate pharmacy records and documentation.
