Requirements

Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD).
Active or eligible for registration with the applicable provincial pharmacy regulatory body
A satisfactory Criminal Record Check and Vulnerable Sector Search are required.
Strong clinical assessment skills, effective communication (verbal and written), and the ability to work independently or within a team.
Experience using electronic health records (e.g., Kroll) and medication order entry systems.

Nice To Haves

Previous experience in a hospital setting or in direct patient care is often preferred.

Responsibilities

Assess patient health, manage medication therapy for chronic/acute conditions, and provide education on medications.
Administer vaccinations (injections), adapt prescriptions, and manage drug therapy to prevent and treat health disorders.
Prepare, package, and distribute medications, ensuring accuracy in dosage, labeling, and storage.
Work within multidisciplinary teams (in clinics/hospitals) to optimize patient care and transition plans.
Act as a drug information resource for patients and other health professionals.
