About The Position

Grand Itasca is seeking a Pharmacist to join our team! This is a Part-Time .7 FTE (56 hrs/pay period) position with a work schedule of Days/Evenings in the Pharmacy department. Clinical pharmacists will work directly with the health care team and patients to provide safe and effective pharmacy services in addition to the dispensing of medications. These services include the provision of pharmaceutical care, continuous performance improvement activities (with an emphasis on Lean Health principles) and education.

Requirements

  • Licensed pharmacist in the state of Minnesota or eligible for licensure.
  • Adhere to the requirements of the Minnesota Board of Pharmacy for licensed pharmacists.
  • Maintain required continuing education for licensure.
  • Ability to manage time and multi-task with many changes in priorities throughout the work day.
  • Critical thinking and complex problem-solving skills.
  • Interpersonal communication skills, both written and verbal, with the ability to work independently in a teamwork environment.
  • BLS certification required.
  • ACLS certification required, or the ability to obtain certification within one year of hire.
  • Compliance with all applicable policies, procedures, codes and standards of Grand Itasca Clinic and Hospital.
  • Demonstrate participation in Grand Itasca Clinic and Hospital's Mission, Vision and Values.
  • Maintains reliable and consistent attendance.

Responsibilities

  • Monitors patient therapeutic responses to medications and adjust medications based on approved protocols.
  • Manages medication therapy with emphasis on GICH's patients with highest acuity (ICU) and greatest risk of harm (pediatrics and neonates).
  • Provides consultative services that foster appropriate evidence-based medication selection.
  • Assists with reconciliation of patients' medications as patients move across the continuum of care.
  • Physician profiling of prescribing patterns by medication and/or diagnosis.
  • Maintains a robust antibiotic stewardship program by reviewing culture and sensitivity results and modifying therapy when warranted.
  • Pharmacokinetic dosing of medications.
  • Manages ambulatory patients through collaborative practice agreements with physicians.
  • Assists in the development of treatment protocols.
  • Identifies, assesses, and manages adverse drug reactions and medication errors with the assistance of the Medication Safety committee.
  • Conducts on-going medication use and disease state evaluations.
  • Provides education, in-services, and competency testing for members of the health care team.
  • Provides patient and caretaker education, including discharge counseling and follow-up.
  • Committee participation representing the pharmacy department.
  • Performs literature based research and medication formulary reviews.
  • Other duties as assigned.
