About The Position

This role involves reviewing and dispensing medications in either a retail/community or medical center/institutional setting. Responsibilities include ensuring prescription accuracy, advising patients and providers on medication use and interactions, monitoring patient progress, and utilizing new technology. In institutional settings, duties may extend to unit dose dispensing, IV admixtures, compounding, and working with automated dispensing devices, while also providing pharmaceutical care across various age groups and adhering to regulations. The role may involve decentralized clinical functions like patient counseling and medication reconciliation, or centralized functions such as dispensing and overseeing clinical operations. Pharmacy services can span across multiple specialties including acute care surgery, neurology, cardiology, pediatrics, oncology, infusion services, intensive care, general medicine, dialysis, transplant, antimicrobial stewardship, and medication reconciliation.

Requirements

Graduate of an accredited School of Pharmacy.
Must have a thorough knowledge of drugs and their actions.
Must have knowledge of state and local regulations.
License must be in good standing by the State Board of Pharmacy in state(s) of practice and/or possess multi-state licensure privileges as required by position.

Responsibilities

Review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability.
Provide information and advice regarding drug interactions, side effects, dosage, and proper medication storage.
Review, interpret and dispense both prescription and non-prescription medication to patients.
Respond to prescription issues and advise providers on the selection, dosage, interaction and side effects of medications.
Counsel patients on medication prescription including but not limited to: proper administration, possible side effects, expected response, storage and any other questions posed by the patient.
Monitor the health and progress of patients in response to drug therapy.
Learn and utilize new technology as the profession of pharmacy evolves.
Provide oversight and clinical supervision for pharmacy services and pharmacy support personnel.
Review and interpret provider orders, provide drug information, assist in problem solving, and provide other aspects of pharmaceutical care.
Perform duties such as unit dose dispensing, IV Admixtures, outpatient prescriptions, navigating the electronic medical record, compounding, and working with automated dispensing devices.
Utilize appropriate age-related patient information to assess the pharmaceutical needs of patients, which may include provision of pharmaceutical care to neonatal, pediatric, adolescent, adult, and geriatric patients.
Follow all established regulations and standards that govern the practice of pharmacy.
Act as a shift supervisor under direct or indirect supervision by department managers.
Demonstrate good written and verbal communication skills.
Involve in acute care surgery, neurology, cardiology, pediatrics, oncology, infusion services, intensive care, general medicine, dialysis, transplant, antimicrobial stewardship, and medication reconciliation.
Engage in decentralized clinical functions, such as direct patient interaction/interdisciplinary team rounding, chart reviews, customer or patient counseling, collecting accurate medication list.
Engage in centralized clinical functions, such as checking and dispensing medications and overseeing the clinical functions of the IV room.
