About The Position
Kindred Hospital Chattanooga is seeking a PRN Pharmacist, with some on-call work required. The Pharmacist supports safe, effective medication therapy as part of an interdisciplinary team by managing drug therapy protocols, reviewing and evaluating medication orders, and identifying, preventing, and resolving drug-related problems. The role also provides medication information and education to medical staff, healthcare professionals, patients, and pharmacy students, and documents interventions and outcomes for drug therapy quality improvement. The Pharmacist evaluates orders for accuracy and completeness before entry and checks technician-filled prescriptions for accuracy.
Requirements
- Graduate of an accredited School of Pharmacy with a pharmacist degree.
- Pharmacy license in the State of Tennessee.
Nice To Haves
- Pharm.D. preferred.
- Experience in hospital pharmacy is desirable.
Responsibilities
- Manage drug therapy protocols as part of an interdisciplinary team.
- Review, interpret, and evaluate medication orders to identify, prevent, and resolve potential or actual drug-related problems.
- Provide drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
- Document interventions and outcomes for drug therapy quality improvement efforts.
- Evaluate medication orders for accuracy and completeness before order entry.
- Check medication orders for accuracy after they are filled by technicians.
