About The Position

The Pharmacist is responsible for providing safe, accurate, and patient-centered pharmaceutical care. This role collaborates with providers and interdisciplinary healthcare teams to optimize medication therapy, ensure compliance with applicable laws and regulations, and support quality patient outcomes. Responsibilities include dispensing medications and other pharmaceutical products, providing medication counseling, and delivering exceptional service to patients and the community while maintaining the highest standards of professional ethics and practice.

Requirements

  • Bachelor of Pharmacy required or Pharm. D. preferred from an accredited institution.
  • Current licensure includes controlled substance license to practice pharmacy in Michigan.
  • A minimum of one year of relevant pharmacy leadership and management experience.
  • Background and knowledge of pharmaceutics and their drugs and interactions.
  • Demonstrated ability to work effectively with people of diverse races, ethnicity, ages, and sexual orientation in a multicultural environment.
  • Basic Life Support (BLS) certification.
  • Knowledge of federal and state pharmacy laws and regulations.
  • Excellent verbal and written communication skills, including strong organizational, detail, managerial and interpersonal skills.
  • Ability to prioritize work and function effectively in a fast-paced environment.
  • Proficiency in Microsoft Office applications and pharmacy management systems.
  • Ability to work collaboratively with interdisciplinary healthcare teams.
  • Ability to travel between Family Health Center locations as needed.
  • Reliable transportation and valid driver's license required.
  • Ability to function effectively in a dynamic and changing healthcare environment.

Nice To Haves

  • Familiarity with the 340b drug pricing program preferred.
  • Immunization certification preferred.
  • Bilingual English/Spanish skills are a plus.

Responsibilities

  • Accept, validate, interpret, and process prescription orders.
  • Accurately dispense medications and provide patient counseling regarding appropriate medication use.
  • Communicate with prescribers regarding medication therapy, clarifications, and recommendations as appropriate.
  • Conduct Drug Utilization Reviews (DUR) and provide necessary clinical interventions.
  • Assist with medication therapy management and medication reconciliation activities.
  • Support formulary compliance, prior authorization processes, and therapeutic interchange programs as applicable.
  • Ensure compliance with all federal and state pharmacy regulations, DEA requirements, and Family Health Center policies and procedures.
  • Maintain accurate records and documentation in accordance with regulatory requirements.
  • Participate in quality improvement initiatives and contribute to safe medication practices.
  • Collaborate effectively with providers, nurses, pharmacy technicians, and other members of the healthcare team.
  • Participate in quality improvement activities for the assigned program area(s).
  • Ensures compliance with all licensure and governing body requirements including but not limited to: Board of Pharmacy, Office of Pharmacy Affairs (including 340B Drug Pricing Program), and Drug Enforcement Agency.
  • Ensures compliance with all contractual requirements of payers including but not limited to: Medicaid, Medicare, private insurance, and all federal, state, and local government contracts.
  • Ensures compliance with wholesale purchasing agreement(s) and maintains close contact with vendor(s) for ordering, inventory and billing purposes.
  • Promote safety precautions and security measures to ensure the safety of both staff and patients
  • Adheres to data security guidelines, including appropriate use of EMR systems and IT resources
  • Perform other duties as assigned.
