About The Position
Bruyère Health is seeking a Pharmacist to join their team. This is a permanent, full-time position with a day shift schedule, working 75 hours per pay period (1.0 FTE). The role involves conducting individual patient assessments, providing recommendations and follow-ups on medication usage, educating patients on medication use, and collaborating with the interprofessional team to enhance medication management programs for improved patient care. Requirements
- Undergraduate degree in Pharmacy
- Minimum of one (1) year experience as a Pharmacist in a patient oriented environment
- Registration as Pharmacist with the Ontario College of Pharmacists
- Hospital pharmacy residency or equivalent clinical experience

Responsibilities
