About The Position

Pharmacist review, validation, and patient specific assessment of medication orders. Monitors patient profiles to assure appropriate dosage and therapy. Provides current drug information. Demonstrates the knowledge and skills necessary to provide care appropriate to the age group of the patients served including participation in pharmacy clinical programs. Possesses the skills and ability to assess data reflective of patient status and to interpret this information and identify the patients' needs in relation to age specific requirements, and to provide care as described in accordance with state and federal law and regulation, and facility policy and procedures. Works under the supervision of the Director/ Manager of Pharmacy. Supervises and performs compounding packaging and dispensing of pharmaceuticals according to prescription, physician order or floor stock request in compliance with state and federal law and regulation. Supervises the work of pharmacy technicians, interns, and students.

Requirements

  • Doctoral Degree, Pharmacy (Required) or Bachelors Degree, Pharmacy (Required)
  • Licensed Pharmacist - Virginia Department of Health Professions (VDHP) within 30 Days(Required)

Nice To Haves

  • 1 year Prior hospital pharmacy experience (Preferred)
  • 1 year Post Graduate Year 1 Residency (PGY1) (Preferred)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) (Preferred)
  • Board Certified Pharmacy Specialists (Preferred)

Responsibilities

  • Supervises and performs compounding (both sterile and non-sterile), packaging, and dispensing of pharmaceuticals according to prescription, physician order, or floor stock request in compliance with state and federal law and regulation.
  • Reviews, validates, and assesses patient medication orders.
  • Monitors patient profiles to assure appropriate dosage and therapy.
  • Reviews alerts associated with verification and escalates concerns to provider or pharmacy leadership as appropriate.
  • Responsible for all core clinical pharmacy programs and commitment to evidence-based patient care, to including, but not limited to IV to PO, renal/hepatic dosing, therapeutic interchange, antimicrobial stewardship, anticoagulation management, duplicate therapy, drug-drug interactions, non-formulary management and weight based dosing.
  • Handles all aspects of pharmacy operations to include, dispensing, sterile and non-sterile compounding, controlled substance/diversion management, pharmacy automation, and all state/federal law and regulations.
  • Establishes an environment of safety and assures safe practices for the patient as evidenced by recognizing and implementing safe medication practices, and following policies and procedures that comply with regulatory agency regulations.
