About The Position

Horizon LTC Pharmacy is seeking a licensed PRN (Per Diem) Pharmacist to join our long-term care team in Belmont, MI. This is a flexible, as-needed position ideal for a pharmacist who wants to stay active in long-term care while maintaining a lighter, predictable schedule. In this role, you'll provide clinical pharmacy services that safeguard patient safety and the effectiveness of therapy—always in line with applicable policies, procedures, and legal requirements. You'll work alongside an experienced team dedicated to the unique needs of long-term care residents.

Requirements

  • PharmD degree from an accredited program
  • Active Pharmacist License and Controlled Substance License (Michigan licensure required)
  • Long-term care pharmacy experience
  • Clinical pharmacy expertise with a focus on geriatrics
  • Solid understanding of pharmaceutical regulations and guidelines

Responsibilities

  • Provide clinical pharmacy services tailored to the unique needs of long-term care residents
  • Collaborate with healthcare teams, including physicians and nursing staff
  • Conduct medication reviews and consultations to support optimal therapeutic outcomes
  • Ensure compliance with regulatory standards and best practices in long-term care pharmacy
  • Dispense medications accurately and provide clinical oversight
  • Direct pharmacy staff engaged in mixing, packaging, labeling, and changes to prescription orders
  • Utilize electronic health record systems for documentation and communication
  • Provide exceptional customer service to patients and caregivers
  • Participate in team meetings to enhance resident care
  • Perform other duties as assigned
