About The Position

The main function of this role is to provide Medication Therapy Management Services. This includes conducting comprehensive medication reviews (CMRs) and targeted medication reviews (TMRs) to assess medication regimens for appropriateness in selection, dose, route, frequency, indication, effectiveness, safety, and patient adherence. The role involves identifying, preventing, and resolving medication-related problems, developing individualized medication action plans, and communicating recommendations to providers and healthcare teams. Additionally, the Clinical Pharmacist will evaluate patient-specific factors such as drug-drug interactions, drug-disease interactions, allergies, renal and hepatic function, laboratory monitoring needs, contraindications, duplicate therapy, and medication affordability. Patient care and education are key components, including providing medication counseling, promoting adherence, educating patients on medication purpose, benefits, adverse effects, monitoring, and lifestyle modifications. The role also supports patients in managing chronic conditions through pharmacist-led interventions and monitoring for conditions like diabetes, hypertension, hyperlipidemia, anticoagulation therapy, asthma/COPD, and heart failure.

Requirements

  • Doctor of Pharmacy (Pharm.D.) degree or equivalent.
  • Active pharmacist license in the state of practice.
  • Experience in clinical pharmacy practice.
  • Knowledge of medication therapy management principles.
  • Ability to conduct comprehensive medication reviews.
  • Skills in patient assessment and problem-solving.
  • Excellent communication and interpersonal skills.
  • Ability to collaborate with healthcare teams.
  • Proficiency in electronic health records (EHR) systems.
  • Understanding of chronic disease management.

Nice To Haves

  • Board certification in pharmacotherapy (BCPS) or other relevant specialty.
  • Experience with specific chronic diseases (e.g., diabetes, hypertension, anticoagulation).
  • Experience in patient education and counseling.
  • Familiarity with population health management strategies.

Responsibilities
