About The Position

Join us as a Per Diem Pharmacist! Hours: Every third weekend 8AM-1PM As a per diem Pharmacist for MiraVista in Holyoke, Massachusetts, you’ll bring your experience and knowledge where your voice matters. A Pharmacist is an integral part of our multidisciplinary treatment team. You will provide direct care and support not only to our patients but also their families and make decisions that impact the lives of your peers, your patients, and your community. In exchange, we will care for you, as much as you care for our patients.

Requirements

• Graduate of an accredited college of pharmacy with a Bachelor of Science (B.S.) in Pharmacy or a Doctor of Pharmacy (Pharm.D.) degree.
• Minimum of one to three years of experience working in a hospital or long-term care pharmacy setting strongly preferred.
• Valid Massachusetts pharmacist license.
• Experience with automated dispensing machines (ex. Pyxis, Omnicell).
• Effective oral and written communication skills.

Responsibilities

• Ensure the delivery of appropriate medication therapy to patients in a timely and safe manner.
• Review medication orders to determine clinical appropriateness and to minimize the risk of adverse events.
• Contribute effectively as a member of a multi-disciplinary health care team to enhance patient care.
• Supervise pharmacy technicians and pharmacy interns.
• Work collaboratively with all pharmacy staff members to effectively manage pharmacy inventory.
• Provide drug information to medical staff and patients upon request or when need is apparent.
• Assist in the maintenance of relevant patient files within the Electronic Health Record.
• Comply with state and federal pharmacy regulations as well as hospital policies to ensure quality of pharmacy services.
