About The Position

Provides patient care through direct operational and/or clinical patient services, ensuring the safe and appropriate use of medications. This role involves drug information, therapy monitoring, distributive, and supervisory pharmacy services, adhering to departmental policies, standard practices, and laws. The pharmacist will communicate regularly with prescribers, office staff, patients/caregivers, and other healthcare professionals. This position also requires meeting departmental staff development requirements and may involve participation in committees and education. The balance between operational and clinical services will vary based on staffing and workload.

Requirements

Bachelor's degree from an ACPE-approved college of Pharmacy.
Licensed to practice as a Pharmacist or eligible to become licensed within 4 months of hire date in the state the team member is working.
Thorough knowledge of medical and pharmacy terminology.
Good mathematical skills.
Good computer skills.
Aseptic technique skills required.
Knowledge of State BOP/Dept of Health/DEA rules/regulations required.
Able to handle urgent, stressful working conditions required.

Nice To Haves

PharmD preferred.

Responsibilities

Provides accurate medication order-entry, drug distribution, drug administration, diagnostic testing, and patient care.
Confirms the appropriateness of drug therapy for specific patient populations (neonates, pediatrics, adolescents, adults, geriatrics).
Minimizes turn-around-time for products and services.
Performs patient education.
Manages patient care.
Reviews and evaluates prescriptions for safety, appropriateness, and clinical efficacy.
Completes prior authorizations/appeals for medications using EMR data and/or direct contact with providers.
Provides initial and ongoing education on medications and associated supportive care.
Creates and monitors individual care plans for patients.
Develops and maintains relationships with medical providers and office staff.
Refers patients to providers, other healthcare professionals, or community resources as needed.
Participates in the pharmacist "on call" program where applicable.
Provides concise, applicable, and timely responses to drug information requests.
Oversees pharmacy support personnel, checking and verifying the accuracy of their tasks.
May function as lead or charge pharmacist to monitor workload and workflow.
Completes lab reviews, pharmacy consults, and provides clinical services with appropriate documentation.
Acts as a liaison between pharmacy and assigned patient care areas, solving distribution problems and monitoring drug storage.
Provides experiential education and training for pharmacy technicians, students, and new team members.
Represents the department in collaborative efforts to integrate pharmaceutical care throughout the patient's continuum of care.
Performs other duties as assigned.
