About The Position

The Pharmacist will be responsible for providing comprehensive medication therapy management services, including conducting medication reviews, assessing medication regimens for appropriateness, effectiveness, and safety, and identifying and resolving medication-related problems. The role involves developing individualized medication action plans and communicating recommendations to healthcare providers. Additionally, the Pharmacist will perform clinical assessments evaluating drug interactions, allergies, laboratory monitoring needs, and affordability barriers. Patient care and education are key components, including counseling on medication use, promoting adherence, and educating patients about their conditions. The Pharmacist will also support chronic disease management through pharmacist-led interventions and monitoring for conditions like diabetes, hypertension, hyperlipidemia, anticoagulation therapy, asthma/COPD, and heart failure.

Responsibilities

  • Conduct comprehensive medication reviews (CMRs) and targeted medication reviews (TMRs)
  • Assess medication regimens for appropriate medication selection, dose, route, frequency, indication, therapeutic effectiveness, safety concerns, and patient adherence and barriers
  • Identify, prevent, and resolve medication-related problems
  • Develop individualized medication action plans
  • Communicate recommendations to providers and healthcare teams
  • Follow up on medication changes and clinical outcomes
  • Evaluate patient-specific factors including drug-drug interactions, drug-disease interactions, allergies and intolerances, renal function considerations, hepatic function considerations, laboratory monitoring needs, contraindications and precautions, duplicate therapy, and medication affordability and access barriers
  • Provide medication counseling and education to patients and caregivers
  • Promote medication adherence and self-management strategies
  • Educate patients regarding medication purpose, expected benefits, potential adverse effects, monitoring requirements, and lifestyle modifications
  • Support patients in understanding and managing chronic conditions
  • Provide pharmacist-led interventions and monitoring for conditions such as Diabetes, Hypertension, Hyperlipidemia, Anticoagulation therapy, Asthma/COPD, Heart failure, and other chronic disease states as identified by clinic needs
