The Pharmacist will be responsible for providing comprehensive medication therapy management services, including conducting medication reviews, assessing medication regimens for appropriateness, effectiveness, and safety, and identifying and resolving medication-related problems. The role involves developing individualized medication action plans and communicating recommendations to healthcare providers. Additionally, the Pharmacist will perform clinical assessments evaluating drug interactions, allergies, laboratory monitoring needs, and affordability barriers. Patient care and education are key components, including counseling on medication use, promoting adherence, and educating patients about their conditions. The Pharmacist will also support chronic disease management through pharmacist-led interventions and monitoring for conditions like diabetes, hypertension, hyperlipidemia, anticoagulation therapy, asthma/COPD, and heart failure.
