About The Position

The Pharmacist, Outpatient serves the needs of UVM health system ambulatory pharmacy patients. The pharmacist may work in one or more areas of the ambulatory pharmacy department, including Retail, Mail Order, Home Infusion, Call Center, Specialty Pharmacy, Travel Clinic, and Transitions of Care.

The pharmacist works with multidisciplinary teams to resolve medication-related problems. They dispense medications according to prescription orders while screening for and resolving medication-related problems and financial barriers, and counsel patients and health care professionals on proper medication use. The Pharmacist maintains good business practices and sound clinical judgment to support departmental success and patient outcomes.

Requirements

• PharmD or Bachelor of Science in Pharmacy degree from an ACPE-accredited college of pharmacy.
• Licensed pharmacist in Vermont.
• Minimum one year of experience in a retail, mail, call center, specialty, or home infusion pharmacy within the past five years.

• Minimum one year of experience in a health system pharmacy.
