About The Position

CVHS is seeking a clinical/operations hybrid pharmacist to work at CVHS- Charlotte Pharmacy. This role involves assisting clinical and operations pharmacy managers with tasks in accordance with Virginia laws and regulations. Clinical responsibilities include performing detailed medication reconciliations, comprehensive medication reviews, and in-person medication therapy management reviews for patients to identify and manage drug therapy problems for positive outcomes. There are also opportunities to work on other clinical pharmacy programs. Operational tasks include overseeing daily pharmacy operations, staffing workflows, scheduling, and distributive functions, ensuring safe, effective, and compliant pharmaceutical care and precise prescription accuracy. The ideal candidate is a team player with strong communication skills, capable of collaborating with providers, nurses, and other clinic and pharmacy staff. The applicant needs solid clinical training, proficiency in pharmaceutical therapy for patient care plans, and the ability to research various drugs, new medications, and guidelines to make informed decisions. A strong work history in the retail pharmacy setting is also required. Requirements

Work as a team player
Strong communication skills that enable them to work closely with providers, nurses and other clinic and pharmacy staff.
Solid clinical training
Well-versed in pharmaceutical therapy to contribute to patient care plans.
Proficient in utilizing resources to research various drugs, including new medications, and guidelines to learn more about their features, uses and side effects.
Strong work history in the retail pharmacy setting.
Demonstrates competence in performing essential job functions.
Demonstrates interest in assisting the Director of Pharmacy Services in the overall operations of the Pharmacy Department.
Sensitivity to the personal feelings and needs of patients and employees.
A desire to interact with the professional staff in promoting a clinical pharmacy program.
Well-developed communications skills.
Graduation from an accredited Pharmacy School with a Bachelor of Science in Pharmacy- or Pharm D.
Licensed to practice pharmacy in the Commonwealth of Virginia.
License is free of restrictions to bill Medicare and Medicaid.

Responsibilities

Assists in supervision of all pharmacy activities as specified by the Drug Control Act.
Provides professional pharmacy services as appropriate.
Performs necessary compounding, using appropriate supplies, equipment and ingredients.
Assists in development of appropriate clinical pharmacy goals and objectives.
Maintains records as specified by the Drug Control Act.
Maintains and uses up-to-date patient drug profiles.
Assists in safe-guarding against diversion of drugs.
Assists in maintaining an up-to-date formulary; uses the formulary to dispense the proper generic or brand-name drug as prescribed by CVHS physician staff.
Assists in selection and control of all pharmacy drug stocks and supplies.
Assists in the administration of pharmacy audit procedures.
Provides patient medication counseling and education through the proper use of labels, accessory labels and personal contact.
Assists in the maintenance of day-to-day pharmacy records as specified by the Pharmacy Director.
All CVHS Pharmacy staff will abide by the standards of accreditation in Mail Order Pharmacy and Specialty Pharmacy.
Attends work as scheduled.
May perform other incidental tasks, as needed.
