The Pharmacist is responsible for the proper dispensing of all medications filled through ExactCare to ensure the correct and safe supply of medications are delivered to the general public. The pharmacist is responsible for adhering to company policies and procedures and providing clinical consults to patients and other health care professionals.

Requirements

Bachelor’s degree in Pharmacy or Doctorate in Pharmacy (PharmD), required.
Must hold a current Pharmacist license in the State of Ohio.
Prior Pharmacist experience, closed-door/production pharmacy preferred.

Responsibilities

Ensuring clinical accuracy and execution of patient prescriptions.
Verification of hard copy and digital Rx’s.
Communication with healthcare professionals and making sure that the Dr. call line is answered.
Supervision and execution of incoming and outgoing prescription transfers.
Perform DUR to identify MTM eligible patients and identify clinical intervention per patient with follow up.
Ensure all legal, privacy and compliance requirements are met.
