About The Position

The Clinical Pharmacist ensures the prompt and accurate provision of pharmaceuticals to patients. Ensure physicians’ orders and patient medication profiles are reviewed, processed, and checked. Drug information is provided to health care professionals and patients to assist in providing rational drug therapy. Supervise and direct pharmacy support personnel daily.

Requirements

  • Bachelor’s degree in Pharmacy and/or Doctor of Pharmacy Degree from an Accredited School of Pharmacy required.
  • Connecticut Registered Pharmacy Licensure (or reciprocation eligibility as deemed by the laws set forth by the State of Connecticut Department of Consumer Protection, Pharmacy Commission for reciprocation).
  • Valid Connecticut Driver’s License required.
  • Working knowledge of Microsoft Windows computer software systems required.
  • Ability to work independently with minimal direct supervision.
  • Ability to work cooperatively with health-system and pharmacy staff.
  • Ability to handle frequent interruptions and adapt to changes in workload and work schedule.
  • Ability to set priorities, make critical decisions, and respond quickly to emergency requests.
  • Ability to exercise sound professional judgment.
  • Ability to communicate effectively (orally and in writing).
  • Ability to meet the pharmaceutical care needs of the pediatric, adolescent, adult, geriatric, and skilled nursing patients.

Nice To Haves

  • Two to three years’ institutional pharmacy practice or ASHP Accredited Residency preferred.
  • Advanced degree and/or certification(s) preferred.
  • Advanced certification in anticoagulation management from nationally recognized program required within one year of hire date (excludes per diem.).

Responsibilities

  • Interprets medication orders accurately to ensure safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.
  • Compounds and dispenses pharmaceuticals, including sterile and parenteral nutrition products, accurately.
  • Maintains all controlled substance transactions records as required by law and issues them to patient care areas on the main campus as well as the satellite unit.
  • Ensures patient receives drug therapy appropriate to his/her age, weight, disease state and diagnosis through course of therapy.
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, unnecessary therapeutic duplication and appropriateness of drug and dose.
  • Assists with pharmacokinetics consult service and drug dosing per organizational protocol.
  • Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.
  • Documents all clinical activities and interventions accurately and completely.
  • Engages in pharmacotherapy literature research and provides accurate adequate and timely drug information to the professional staff, patients and their families, and employees upon request.
  • Provides patient discharge counseling and documents such in progress notes of patient chart.
  • Responsible for providing coverage after hours as scheduled via the on-call system.
