Bargaining Unit: UUP Job Summary: The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking a full-time Pharmacist. The successful candidate will: Dispense, compound, admix drugs and other pharmaceuticals. Provide medication counseling and reconciliation. Review medication orders for safety and efficacy and then approves orders within our CPOE system to main medication profiles. Review and approve Pyxis profile orders. Administer oncology, TPN, and investigational drug solutions as needed. Dispense controlled substances as needed and maintain files. Participate in drug utilization review studies, QA, and PI programs. Actively participate in new technologies and operational programs. Maintain the records of Pharmacy. Supervise technicians and support staff. Report to Senior Pharmacists, the Director, and Associate Director of Pharmacy. Provide in-service education to healthcare professionals on demand. Rotate with medical teams and/or participate in specific medication management programs. Be available for holiday coverage rotation and on-call/recall rotation as required.

Requirements

New York State Pharmacist Licensure.
Pharm D. from an Accredited College of Pharmacy

Nice To Haves

Prior hospital Pharmacist experience.
Completion of a PGY-1 Residency.
