About The Position
The Pharmacist in our Transitions of Care unit at Cedars-Sinai Medical Center in Los Angeles, California will assist in overseeing the daily organization and supervision of transitions of care medication management activities in their areas, including but not limited to provision of clinical and drug distribution services and other duties as assigned.
Requirements
- Must have a valid California Pharmacist license.
- Doctor of Pharmacy (PharmD) degree.
- Completion of a PGY-1 Pharmacy Practice Residency or equivalent experience.
- Experience in transitions of care or clinical pharmacy.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
Nice To Haves
- Board certification in pharmacotherapy or a related specialty.
- Experience with electronic health record systems.
- Experience in a large academic medical center.
Responsibilities
