The pharmacist-specialty is a hybrid clinical pharmacist and sterile compounding lead position. The pharmacist – specialty must: Be proficient in accurately verifying medication orders and dispensing medications Assures safe, effective, and economical use of medications Reviews and evaluates medication orders Monitors patient medication therapy Effectively and appropriately communicates with providers Safe storage, preparation, dispensing of medications Provides medication counseling for patients and drug information for providers Participates in medication event reporting Offers leadership to pharmacy technicians and precepts students and/or residents (at least 1 annually) and new hires Facilitate inventory control Offers drug information support to pharmacy staff Consistently supports daily pharmacy workflow by answering operational questions Covering on call when necessary Be proficient in the following clinical pharmacy services Consult services including medication reconciliation Renal dose adjustment and vancomycin monitoring Active participation in interdisciplinary rounds Contributes to development or review of clinical or operational policies as assigned Active participation in local or regional multidisciplinary or pharmacy specific committee or workgroup Be proficient in the following responsibilities of USP 797 Designated Person Ensure overall compliance with USP 797, applicable federal and state laws and regulations, accreditation standards and Trinity policies Updating/creating standard operating procedures Oversight of personnel training and competencies Training sterile compounding staff and monitoring and observing sterile compounding activities
